School form for document verification

If your child goes to school in league boundaries, the school form is the easiest option for address verification as it is only the form is required in addition to the players original or certified birth certificate.

In order to play for Oceanside National Little League, players must either reside in or go to school within our boundaries. If choosing the school option, you must submit the School Enrollment Form signed by a School Administrator, Principal or Vice Principal. Please print out the pdf below and submit to your school for verification.
